Adoption Questionnaire

To adopt an animal, you must be at least 18 years of age or older. The following information is requested in order to help you select a pet most compatible with your family and lifestyle. Our number one priority is to place a pet in a permanent and responsible home. Pets deserve to be a loved and respected addition to your life. The Nelson County Animal Shelter reserves the right to refuse adoption if determined to be in the best interest of the animal. Please complete the questionnaire. You may fill out either the cat or dog section depending on which animal you wish to adopt. If you would like to adopt both a dog and a cat, please fill out both sections.
